How to request an appointment

Due to recent changes in how General Practice works, we have moved over to the Total Triage Model of working for requesting both urgent and routine appointments. This allows us to assess every request based on clinical need and signpost to an appropriate appointment or service.

For medical emergencies please call 999 or attend your nearest A&E

The easiest way to contact us for an appointment is via our online form.

The forms are available from 8am each weekday morning and the link can be found here.

Please complete the form and add as much information as you can when asked. This will then be assessed by a GP who will decide the best course of treatment for you and the urgency of your request. We will respond to your request the same working day.

Should you be unable to complete the form, please contact our reception team on 0114 245 6370 and they will complete the form with you.

Our online triage service hours are:

Monday – 08:00am to 6.30pm

Tuesday – 08:00am to 6.30pm

Wednesday – 08:00am to 6.30pm

Thursday – 08:00am to 6.30pm
